A completed Statement of support from someone that knows can be given to Social Security Scotland to help them understand your care needs better.

If you're applying online for Pension Age Disability Payment, you should:

Download the PDF on this page.
Give it to the person you want to complete it.
Ask them to return it to you when it's been completed.
Upload it as part of your application online, or by using Social Security Scotland's document upload tool.
