If you cannot verify identity online
As part of the application process your identity will be verified online.
If this verification is unsuccessful, you'll need to send Disclosure Scotland a copy of two identity documents, which confirm your date of birth and address.
Documents to confirm date of birth
You'll need to provide a copy of one of the following, which includes your date of birth:
- passport (photo page only)
- driving licence photocard (full or provisional)
- biometric residence permit
- national identity card from a country outside the European Economic Area
- birth certificate
- adoption certificate
- marriage certificate or civil partnership certificate
- NHS card
- UK armed forces identity card
- UK firearm or shotgun certificate
Documents to confirm current address
You'll also need to provide a copy of one of the following, which includes your current address:
- utility bill, such as gas, electric or phone bill
- council tax bill or letter
- driving licence photocard (full or provisional)
- bank or credit card statement
- mortgage statement
- tenancy agreement (typed, not handwritten)
- insurance policy document or letter
- payslip or P60
- UK hospital or doctor's letter
- UK TV Licence
- university, college or school letter
- UK Electoral Register Office letter or UK polling card
- Home Office letter
- UK firearm or shotgun certificate
- UK solicitor's letter on headed paper
Your proof of address must be no more than 12 months old, although your driving licence can be older. You cannot use the same document for both date of birth and address.
You can email or post copies of your documents. Do not post originals, as they will not be returned.
Email your documents
Send photos of the documents or scanned copies. Include your full name and reference number in the email. Email it to response@disclosurescotland.gov.scot.
Post copies of your documents
If you cannot email your documents, post copies to:
